Avoid use of deprecated functions + fixed package installation.

By default, the compiler is not invoked directly (can be overridden using --rdmd).

Fixed up the VisualD and Mono-D generators and changed their names on the command line to lower-case.

Added a new "build" generator that directly invokes the compiler. ...

This generator supports all compilers in contrast to the rdmd based one and will later be the default.

Moved empty package creation from app.d to the Dub class.

Changed the initial project source code (dub init) to not depend on vibe.d.

"dub build" and "dub run" are now also executed using the generator system (RdmdGenerator).

Also added support for gdmd and ldmd

Dub builds with VisualD with latest vibe. BuildPlatform is passed from app.d to the generators. System libraries are suffixed with '.lib' for VisualD.
